MY STORY
Isabella's Gold Award Project
Who
Isabella Gonzalez is currently a senior attending Peachtree Ridge High School. She participates in the Peachtree Ridge High School Colorguard team, and loves to volunteer in her community.
What
Isabella is also an Ambassador in Girl Scouts and is currently working to complete her Gold Award. A Girl Scout Gold Award is the highest level of honor in Girl Scouts and if completed looks amazing to college admissions. Her project is in support of seniors living alone with possible medical conditions. She has been working alongside in a partnership with “Mending The Gap”, in order to help her distribute her project. Her project is “Plan In A Can”, a recycled tennis can to be stored in a secure, yet easily accessible location in the patient's home. The can will contain a medical form the patient has completed. The can will also hold personal items including a copy of their driver's licenses and insurance, reading glasses, chap-stick, medical directives, copies of their prescription labels, etc. To watch an in-dept video on the process of making cans follow the link at the bottom of the page.
Why
In the article, Emergency hospital admissions among older adults living alone in the community, it states, "Older adults living alone were at higher risk of Emergency Department (ED) admission and higher inpatient costs regardless of multimorbidity, while those living alone without multi-morbidity had the longest average inpatient days. To enable aging in place while avoiding ED admissions, interventions could provide instrumental support and regular health monitoring to older adults living alone, regardless of their health status.“ The lack of electronic medical records in rural communities makes it challenging to properly treat elderly or disabled people who live in rural areas. The Plan In A Can could be a solution to one of the problems described in the reference above.
How Can I Help?
When Christmas time approaches, we plan to give seniors a little gift bag consisting of a $10 gift card, socks or lotion, a Plan In A Can, and a handwritten letter from younger girl scouts. In order to give these seniors this amazing gift, we need to fundraise money to afford all of these items. Thus, we are selling cookie jars for $10 each in order to raise money. We have a wide selection of cookies to choose from. If you would like to place an order, please click “Store” at the top of the page and fill out all of the questions.
Can I Get A Plan In A Can?
Absolutely. Depending on your location, will depend on the days it will take to deliver. To request a can or multiple cans, please email isabella4gonzalez@gmail.com. In the email, please include your name, address, and how many cans you would like to have. All cans are free of cost.
